Filling Out a Form

Servers use forms for staff applications, event sign-ups, surveys and ban appeals. A form is a web page on the dashboard, so you fill it in in your browser, not in Discord.

Find a form

Staff usually post a link to it in a channel.
Run /form list in Discord to see the server's open forms.

A form's address looks like /servers/<server>/forms/<form-name>.

Fill it in

Open the link. The page shows the form's name, the server and a description.
If the form needs you to be logged in, the page says so: log in with Discord, then open the form again. Some forms can be filled in without logging in.
Answer the questions. Required questions are marked with a red *. Text answers show how many characters you have left, dropdowns start at Select an option..., and some questions offer Other... with a box to type your own answer.
Click Submit.

If a required answer is missing or out of range, a message at the bottom right of the page names the question, for example: Please fill out "Which timezone are you in?". Fix it and click Submit again.

When it is sent, the page shows Form Submitted.

After you submit

Staff review the response in the dashboard. They may vote on it, then accept or deny it.
Depending on how the server set up the form, you may get a DM when your response is accepted or denied, a role when it is accepted, or an announcement in a channel.
Some forms let you change your answers while they wait for review. Open the form again: under Your Submissions, use the pencil to edit (then click Update Response) or the bin to withdraw it.

Why you cannot submit

The page tells you the reason. The most common ones:

Message	What it means
You must sign in with Discord to submit this form.	Log in, then open the form again.
You must be a member of this server to submit this form.	Join the server first.
You have already submitted this form.	This form takes one response per person.
You are on a reapplication cooldown.	Your last response was reviewed recently; the page shows when you can apply again.
This form is currently closed.	Staff closed it.
This form closed on (date) UTC.	It had a closing time, and that time has passed.
This form has reached its maximum number of responses.	It takes no more responses.
You can no longer submit this form.	A previous response was permanently denied.

Ban appeals

A server can use an appeal form so banned members can ask to be unbanned. When you are banned, the bot's DM may include a link to it.

You must log in with Discord, and the form only accepts members who are currently banned from that server.
You can have one appeal waiting at a time. After you submit, the page reads Appeal Submitted, and you get a DM when staff decide.
If an appeal is permanently denied, you cannot appeal again.

Your answers and your privacy

Your answers are stored with the server that owns the form, and its staff can read them. If you delete your account, the server keeps your answers but they are no longer linked to you. See Your Account, Privacy and Data.
