Forms - Setup
This walkthrough creates a feedback survey. Applications and appeals use the built-in forms instead; see Applications and Positions and Ban Appeals.
- Check the module is on. Open Forms. The switch at the top of the panel must be on (it is by default).
- Create the form. Click New Form. New forms are General forms; the Introduction, Application and Appeal types are the three built-in forms, which you edit and switch on instead.
- Basics. Fill in the Title (up to 100 characters) and an optional Description (optional). The URL Slug follows the title until you save; it becomes the end of the form's link and must be unique in the server. Leave Enabled on.
- Questions. On the questions tab, add your questions: pick a type, write the label, set Required and the type's options. See Question Types.
- Settings. On the settings tab, choose who can answer and how often (for example One Response Per User), a Max Responses limit, and roles to give on completion. See Form Types and Settings.
- Channels. On the channels tab, pick a Completion Announcement Channel if you want a card posted for every submission, and its Submission Template.
- Save. Click Create Form (Save Changes when you edit an existing form).
- Share it. Click the link icon (Copy public URL) on the form, or run
/form link slug:<slug>in a channel to post a card with a button named Open and the form title. Members can also find enabled forms with/form list.
The form page works on phones and desktops. Members who are signed in see their earlier responses there, can withdraw one that is still pending, and can edit it where the form allows editing.
The order of custom forms on the Forms tab (the arrows) is the order /form list shows them in. The switch on each form turns it on or off; a form that is off answers "This form is currently unavailable." and is left out of /form list.
