Forms - Overview
Forms lets you publish web forms for your server: surveys, introductions, staff applications and ban appeals. Members fill them in on the dashboard's public form page; staff review applications and appeals from a card in Discord or in the dashboard, vote on them, and accept or deny them. Accepting can give roles, and accepting a ban appeal unbans the member.
The four form types
| Type | For | What is special |
|---|---|---|
| General | Surveys, feedback, sign-ups | Anyone can answer, by default without signing in and any number of times |
| Introduction | "Introduce yourself" posts | Sign-in required; a member can edit their introduction |
| Application | Staff and team applications | One pending application per member, a re-apply cooldown, optional positions, a staff review card with voting, roles on acceptance |
| Appeal | Ban appeals | Only banned users can submit; accepting unbans them; appeals can be permanently denied |
Each type starts from its own defaults and locks the settings that do not fit it; see Form Types.
Every server starts with three built-in forms, all turned off until you enable them: Introductions (/introductions), Staff Applications (/applications) and Ban Appeals (/ban-appeal). Built-in forms can be edited and turned off but not deleted. Click New Form for more.
How it works
- You build a form in the dashboard: basics, questions, (for applications) positions, settings and channels.
- You share its link, or post it with
/form link. Its page is/servers/<server>/forms/<slug>on the dashboard. - A member fills it in. The bot can give roles on submission and post a card in a Completion Announcement Channel.
- For applications and appeals, a review card appears in the review channel with Accept, Deny and, when voting is on, Upvote and Downvote buttons. Appeals also have Permanently Deny.
- Staff accept or deny, from the card or the Responses tab. The member gets a DM with the outcome.
The module switch at the top of the Forms panel turns everything off at once: every form page answers "This form is currently unavailable.", /form list and /form link stop working, and the ban DM stops carrying the appeal link.
